Tubes....


Case 1: RTA patient came in with cervical spine injury and patient was intubated. CXR shows no pneumohemothorax, further confirmed by Sx team. Later, patient deteriorate suddenly, so found out patient had pneumothorax. Chest TUBE inserted 2-3 times, and finally the third time, there was +++ bubbling. Subsequently, patient proceed to cervical spine CT, which also noted Chest TUBE was actually protruded to lung parenchyma. So, chest TUBE was removed, and inserted again, still not too well functioning. Patient was sent to ICU, and chest TUBE was inserted again and again, 3 times, by anaest in ICU, because of difficulty in insertion, finally the third time, chest tube functioning well. Subsequently, patient did quite well, and slowly recovered, able to be discharged from ICU to a normal ward.


Case 2: A para-suicidal patient, ingested dont know what medicine, query PCM, came to A+E, drowsy, V/S otherwise stable. Ryle's TUBE inserted and gastric lavage with large amount of Charcoal carried out. Suddenly, patient deteriorate, much charcoal came out from her mouth and nostril. Intubation was needed to secure the airway, but it was real hard cause everything we see inside the throat are covered with blackish charcoal, tried a few times by the A+E MO. Anest called in for help in intubation which was successful intubation, and noted by them, actually Ryle's tube was inside the trachea-lungs.


Case 3: BPH patient, came to A+E with hematuria and blot clots noted in CBD and urine bag. Was discharge back home and hematuria is actually blood stained urine. CBD was due off but since patient had hematuria, he needs to keep the CBD and so CBD was changed. Next day, patient came back in again with frank hematuria, it was real maroon blood color ?urine. I think it's blood coming out, not urine anymore.

Lessons to learn:
1) Tubing looks easy, but may cause severe complications.
2) Ask/ consult others always if something isn't fully sure.
3) Dont ever hurt yourself, and come to hospital, cause once you're in hospital, you are our partial experimental material.
So, love yourself, keep healthy, keep yourself away from Hospital.

Every time I see patients coming in to hospital because of so and so complaints, reminds me to be so thankful and grateful to God for what I have and I am now.....

Thank You for my good kidneys, heart, brain and all others internal organs,
Help me to keep myself healthy and well all the time.
I HATE to be in Hospital as a patient.
